My optician always insists taht I have had my lenses in for at least 3 hours before the exam so he can see what my eyes look like after I've worn them a while. Its also a good idea to wear lenses that are just before the time you need to throw them out. My optiican likes ot look at the contacts in teh worst scenario and also some like to put yellow dye in your eyes to check fit and other things and then you end up with yellow contacts. Also however clean my optician is I don't like other people handling my contacts so I prefer to chuck them afterwards.
Either way; we all have our own personal preference, and the doctor or staff will let you know when you get there. If you're having problems with them, I'd say definitely wear them in so the doctor can see what the lenses are doing, and otherwise it's entirely up to you.

Either way. An optometrist or an ophthalmologist can pop the lenses in or out faster than you can ask, "Should I have my contacts in or out?".
Yes. They will examine your eyes with your contacts in first then you will have to take them out and they'll examine them without them in.
I wear contacts
You won't need them at all. Don't wear them, you'll only have to take them off when you get there.
