Question: we found this used by a nurse not familiar with our policy in our hospital. we figured it out after about fifteen minutes. it was found at the end of each entry before the signature. it is not in our list of approved abbreviations.

The symbol is self made. Hospitals are getting away from using symbols because they can leave room for mistakes.
There are " approved " abbreviations that are to be recognized by others'. However, sometime healthcare providers make up their own abbreviations. Kind of like text coding. One of the purposes for medical abbreviations, was to reduce the amount of time in writing " Nurse's " notes or notes from other medical staff. However, due to an increase in medical errors, "once approved " abbreviations are no longer used to prevent misunderstandings. And, today, many hospitals or Doc's offices use computers or electronic charting.
