How that works: The practitioner would use a scalpel to make some incisions in the gums, then flap them back for access (detach the gum from the underlying bone). This will give the practitioner access to the tooth roots and deep pockets so the crusted calculus (hardened tartar) that was deep beneath the gumline can be scraped off. Once all the exposed calculus are cleaned off, the gums are sutured back together.

This is done for patients who are found to have deep periodontal pockets that cannot be cleaned out with just scaling and root planing, and its purpose is to halt the progression of periodontitis before it starts to cause pathological mobility in the teeth.
