No you definitely don't have lung cancer if you've only smoked 3 - 4 months and you're still young. It takes a lot of smoking to get lung cancer. Lung cancer usually occurs in older people after decades of smoking. You could be getting some irritation in your oral mucosa. Could be due to the smoke and dryness. You should stay well hydrated and make sure to drink water and stuff.
It sounds like you could also be having an upper respiratory tract infection.. nothing serious.. usually a virus. It should go away on its own after a week or so. If it doesn't after a week, it could be a bacterial infection.. still nothing serious.. you will just need antibiotics for it.
by the way, you should be open with your doctor and you should be able to tell him anything. he won't tell your parents about your smoking.

im young and ive been smoking for 2 years not the best choice but its an alternative for me from other bad substances goin around alot anyway i have had the same problem as u, if ur body cant take it dont do it otherwise u will make it worse in your case probs just agitated tissue from smoke.

You need to cut down smoking and get proper treatment.
