Blood letting was one use, thinking diseases were caused by plethora or excessive blood. In modern days, they were used to produce Heparin, the anticoagulant or the drug that prevents blood clotting. It has many useful purposes.

To suck out the infection.

They still are used today! I was surprised when I worked in a pharmacy at a hospital and we had to order them in! They have certain properties and they actually filter the blood or something like that.

Leeches were often used when a person had a fever, as the common idea was at the time that you had too much blood in your body, thus the also popular practice of bloodletting(opening a vein and bleeding a patient, generally until they grow pale). They were also used somewhat to attempt to extract infections from the body.
