paranoid i thing everyone is against me, i do not remember any thing,can't pay any attention to anything always spaceoff thinking about the same thing over and over,no motivation??????scared to go to counseling

Any number of things could be causing that, and all we could give you here would be educated guesses. I do encourage you to talk to your doctor about this whether you decide to go to counseling or not. There might be some medications which can help you.
Maybe if you had a friend or other loved one with you at the appointment, you might not feel so nervous...?
