A large study in Europe and North America found 32 deaths due to codeine. In addition codeine has a median lethal dose (LD50) of 266 mg/kg. So for every kg a person weighted they would need to take about 266 mg of codeine and even if they take 266 mg for every kg their risk of death is 50%. There have been deaths at about1,000 mg but they are uncommon especially since a normal dose of codeine is 30-60 mg
It is actually not common for people to die after overdosing on medication, especially codeine which is metabolized in such a way to make it much safer than most opioids.
The rate of addiction is almost none. Medical use of codeine rarely leads to addiction, even the VAST majority of Heroin users are not addicted. The most liberal estimate is that a person taking codeine recreationally has at most a 1% risk of addiction.

It's extremely rare too die of codeine overdose. A logical guess would be 100 deaths in the entire world in one year. Most definitely not 3000.
It's extremely rare to become addicted to codeine.
I suggest doing a drug report on Oxycodone (OxyContin) It's about 40x as powerful as codeine and is extremely addictive, nicknamed "Hillbilly Heroin"
