the only way you are not considered a virgin anymore is when you have sexual intercourse. Most girls have a hymen, which is a flap of skin located right on the outside of the vagina and partially covers it. Some girls are born without one, and everyone's is different. Some girls hymen's are torn during sex and some are torn due to other activities, although some are not torn, even during sexual intercourse, merely stretched. So in all technicalities the doctor cannot tell whether you are a virgin or not, compared to just using tampons, or if you have masturbated, and even if they could tell that easily it is confidential between you and the doctor, your doctor cannot tell anything to your parents if you ask them not to.
