Its not that they know where to target the pain. NSAIDs enter your bloodstream within 20-30 minutes, thus circulating the medication throughout your body. Also, ibuprofin does not alleviate pain per se. What happens in the case of inflammation, the pain is caused by the inflammed area, the ibuprofin targets inflammation, and once the inflammation is gone, the pain dissipates. They are not actually considered pain pills contrary to most peoples' beliefs.
