This electrodiagnostic examination of the lower extremities is mildly abnormal.

There is evidence of mild distal axonal sensory neuropathy consistent with the patient’s history of diabetes.
Patient has a history of direct trauma to his lower back with probable soft tissue, bone, and joint injuries that has given rise to referred pain down his lower extremities, without causing significant injury to the exiting spinal nerve roots.

The patient has concerns of possible urinary incontinence symptoms which may need appropriate urological evaluation.

Sounds to me like you have what I have nerve neuropathy. Which is where the nerves are inflammated and cause alot of pain. Ask your doctor if they will prescribe Lyrica which is great for nerve pain! I have it and it helps!

You need to ask your doctor, but basically, you have mild nerve damage caused by diabetes and back trauma, and any possible urinary incontinence symptoms need to be looked at by a urologist.
