i have the same problem! i know it sounds stupid, but if i put one hand over my eyes (to make it seem dark) it makes it easier for me. for some reason it makes me feel like there is no one else there.

yes, i get stage fright sometimes, but i just hover there until i can go.

Yes I get performance anxiety too. I just try and relax and not think about it so much. The more self conscious I get the harder it is.
