What do I write on a medical card in case of an emergency?!

What type of information should I write on my medical card?
Any medications you are on. What type of insurance you have. Who is your primary physician and his/her phone number. What hospital is your preference. If you have any diseases or conditions. Also, an emergency contact, like a friend or family member to be contacted in case of an emergency.
If you have a living will or DNR (do not resuccitate) then you should also post your lawyers name and number. Or just note that your hospital or doctor has a copy of your living will or DNR
And who to contact.
As I am a paramedic...any communicable diseases you may have, the chance if you could or may not be pregnant, how much you weigh, any allergies, and any other diseases/medical issues you might have. Anything else is useless and really wouldn't matter anyways.
Your name - So EMT's know what to call you by so you can respond to them
Number of someone they can call
Types of medications you take regularly and the dosages. Both Rx and OTC meds.
Any chronic conditions you have - ( Asthma, Epilepsy, CHF)
Any major surgeries - ( Gastric bypass. etc.)
Your religious preference ( so they know if they should contact a Rabbi or Priest if they need too. Some religions forbid blood transfusions)
Any last wishes you may have - ( ie, donate organs)
