Sometimes it is necessary to flap the gingiva to give better access so we can work an elevator tip into the periodontal ligament space between a tooth and the bone it's sitting in, so as to loosen the tooth and pull it out.
If the gingiva is flapped for access (detached from the bone underneath) or accidentally torn in the course of the procedure, the guy doing the extraction should place sutures to close it back up once the extraction is done so that the site will heal properly.
