It simply depends on your situation. My orthodontist began using a PowerChain with my braces probably about 5 or so months into treatment. Towards the end, he used them on my upper teeth because they were already where he wanted them to be. So, it honestly really depends on your situation and your orthodontist's personal choice.

About an hour. A little tip - take a couple of ibuprofens before you go in - then it won't hurt so bad when they've finished!
Not that long actually. It only takes about a few minutes, and your ortho will wrap each one every bracket. You will get power chains once your teeth are in their proper positions and if your ortho thinks you ready to fill in the gaps between your teeth. I hope this was of some help. P.S. It really depend on the situation, I mean I saw some people who got them the very first month they got braces, and others who got them several months into treatment and towards the end. It varies.
