A periapical lesion is a lesion found on the apex of the root of a tooth/ very tip of the root of the tooth, deep down in the bone. Usually caused by a cavity that has gotten into the nerve space, this spreads down the root and exits out the apex of the root letting infection pile up around the tip of the root. At this point, a root canal is needed to fix this or an extraction.

As the hygienist has stated, periapical means at the end of the root of a tooth. This lesion [destruction of bone] can be due to an infected tooth, cyst, or tumor and the treatment would depend on what the lesion was.
