I think you're going to need professional help with this, to get him out of that cycle of bad eating. It's more than just eating now, you realise this ....
It might be an idea to find ways of encouraging him to take an active part in preparing his food, let him cook with you, and talk about things he likes. This way you can show him the various different ways of preparing things. Helping him understand about how his food is prepared and works for him can help him make decisions that will help him - which is very empowering for a child who needs confidence.

speek to his doctor about his weigh issues and ask about what sort of diet he should be on.....hes sooooooo young......he needs to learn healthy eating habits and it cant be some crash diet or adult diet...needs to be for him!
