Rule of thumb for maximum heart rate is 220 minus your age. If you are about 20 years old, your maximum heart rate is probably around 200 bpm or so. This varies from person to person, of course, but that's a standard ballpark figure.
Just as a rule of thumb, you subtract your age from 180 to get your target heart rate during cardio work. So you should not be above 160--maybe if you were in great shape and training for a race you might shoot for higher, but not generally.
Maybe you just didn't realize how hard you were working on the ELLIPTICAL machine. It's a low-impact/no impact machine so it's easy to get fooled about how hard you're going. But it also could be that there's something wrong.

yeah smoking could boost your heartrate with less activity your doing. Cardio and smoking does not mix :/
