There are many features that will determine your best treatment - you really need to sit down with your doctor and discuss this!. It takes a complete review of your medical history, labwork, and physical exam to determine your treatment!.

If you have the growth of a fungus ball in the sinus, it should be removed surgically!.

If you have evidence of bony erosion, this means that you have invasion of the fungus into the bones, and in the sphenoid sinus this is very serious -- you are only a few millimeters away from the brain!. Again, surgical debridement would be best, followed by a course of antifungal drugs!.

It would be a judgement call on how to treat you with medications!. Probably the best choice is voriconazole, but you might require (IV) amphotericin --- much of this depends upon WHICH fungus is growing!. Some would not respond to voriconazole!.

You probably have a very complex medical history that led to this problem -- you definitely need a very thorough consultaion with an ENT surgeon as well as an infectious disease specialist to give you responsible advice based on factors that are just too complex to address here!.

Please see both of those specialists for the best advice !
