smallpox is caused by a virus (Variola major) that was irradicated by a world wide vaccination effort. it officially still exists in two laboratories--at the CDC (america) lab and VECTOR (in russia). there is a legitamate fear that other labs and organiztions may also have smallpox samples.
the 1978 outbreak involved a medical photographer who worked a floor below a smallpox research facility in england that wasn't taking every recommended precaution. it was extremely small and contained. the photographer died of smallpox and her father had a heart attack.
The disease has been eradicated as far as we know through vaccination. The bacteria is maintained in some very secure bio-laboratories.
