Yes, You will have to see a dermatologist and he may refer you to a plastic surgeon. If the Dermatologist feels you need to have it removed Insurance will cover it. Talk to the dermatologist and tell him it itches and sometimes bleed. Most times when you say that the Dermatologist will remove it or refer you to a plastic surgeon. If you tell him nothing he may not remove it and the Insurance won't cover the removal. ....From one who had some moles removed. They also biopsy the mole and send it to the lab to make sure its not cancerous.

Insurance should cover the removal because the mole has changed in size...

The reason they should cover that is because any doc worth their salt after you tell them that will remove it to make sure it is not cancer.
