Can't really say what it would be like for you... you may hardly feel the needle going in or it may be the most painfull experience of your life. Everyone will feel thing a little different. I have a Jacob's ladder myself and while it is not a scrotal piercing the basic idea is still the same, and no it really wasn't that bad getting them done.
I pierced a boyfriends sack once. He made me do it over and over until I got it in just the right place. I thought he was crazy, but I got sort of turned on by it.
why would you want it pierced? i dont think that is a spot where i want a sharp metal object going
